5G Network strongly depend upon ultra dense integrated small cell beside macro cell. This kind of UDN consists of large number of macro cell and small cells through which increase in energy demands can be met. Practically energy consumption is done by dynamically controlling modes in radio networks. This paper reflects the significant power saving by considering only a subset of network node which reduce traffic migration and control phase signaling. To enable the sustainable UDGN improve system energy efficiency, and alternative energy source are introduce to reduce dependence on traditional fossil fuels. In 5G optimizing energy is the most important challenge and maintenance of RF communication system. RF energy harvesting is a unique technique for 5G system as an alternative to traditional energy supply sources. Future evolution of the networks has also been assumed to open many new business horizons for the operators and the need of not only a resource efficient but also an energy efficient ecosystem has greatly been felt. The deployment of small cells has been envisioned as a promising answer for handling the massive heterogeneous traffic, but the adverse economic and environmental impacts cannot be neglected.

In the era of globalization, which is marked by intense competition between hospitals, it requires anticipating improvements in service quality on an ongoing basis in line with the increasingly high expectations of customers for hospital services. This paper presents the quality of internal services and individual characteristics of satisfaction and its implications for employee performance. The population of this study was 56 participants of the Regional General Hospital (Korpri) of East Kalimantan Province through a questionnaire. The analysis model uses Structural Equation Model (SEM) based on Partial Least Square (PLS). Overall, the Quality of Internal Service has a positive and significant effect, while Individual Characteristics have a positive and significant impact on Job Satisfaction. The second model proves that the Quality of Internal Service actually has a positive and significant effect on Employee Performance. On the other hand, Individual Characteristics and Job Satisfaction have a positive impact, but not significant for Employee Performance. In order to improve employee satisfaction and performance through individual characteristics and service quality, there is a need for support from management and fostering good relationships with superiors and colleagues.

Introduction: Prostatic adenocarcinoma is one of the most frequently diagnosed cancer in adult males which seldom metastasizes to testis. Method: In current study the authors retrospectively analysed the 154 cases of bilateral orchiectomy done as hormonal ablation therapy in cases of prostatic adenocarcinoma. Result: We received a total 154 bilateral orchiectomy specimens of patients with prostatic adenocarcinoma in the period of three years. The Patients presented were from 42-86 year of age with a mean age of 60 years. In most (83.11%) of the cases testes were atrophied, 15.58% cases presented with normal histology and only two cases (1.30%) were reported positive for metastasis in testicles. Interestingly both of our cases also had a low PSA level. Conclusion: Testicular atrophy is the most common histopathological finding in patients of bilateral orchiectomy done for prostatic adenocarcinoma. Testicular metastasis is very rare but its possibility highlights the need for continued monitoring of patients with a history of prostatic cancer for local recurrences as well as metastatic disease. It also shows the importance of detecting metastasis to rare locations such as testicles specially in the setting of low PSA levels.

When African countries gained independence, most African political leaders turned their back to their populations, and African male witers changed their topicality to political disillusionment. African female writers, included Buchi Emecheta, could no longer identify themselves with men’s writngs and started denouncing the hard linving conditions of women, hence The Joys of Motherhood (1979). After gender preoccupations, Emecheta publishes The New Tribe (2000) to address self-discovery. This article displays how Buchi Emecheta has shifted from “gender preoccupations” by castigating the irresponsible and incompetent fathers of family who do not take care of their wives and children to “universality”.

Metabolic syndrome (MetS) is increasing globally. However, there is paucity of information on its association with obesity amongst young adults in Nigeria. This study was designed to investigate the prevalence of obesity, metabolic syndrome and the different BMI-metabolic risk sub-phenotypes in a young adult Nigerian population. A total of 200 young adult Nigerians (92 males; 108 females) took part in the study. Blood pressure and anthropometric readings were taken following standard protocols and body mass index was determined. Fasting blood sample was collected and biochemical assays were done using standard protocols. Obesity and metabolic syndrome were defined by the World Health Organization and modified National Cholesterol Education Program Adult Treatment Panel III criteria, respectively. BMI-metabolic-risk sub-phenotypes were defined by the presence or absence of the metabolic syndrome within the 3 BMI groups. Obesity was found in 10.88% (5.40% males; 15.80% females) of the study population. Metabolic syndrome was found in 23.80% (27.58% males; 20.58% females) of the population while hyperglycemia (41.26%) and hypertension (36.50%) were the most common metabolic abnormalities. Amongst the overweight and obese population, 80.00% and 60.00% respectively had healthy metabolic profiles while 23.68% of the normal weight participants were metabolically unhealthy. The different BMI-metabolic-risk sub-phenotypes occurred at rates of 20.00%–80.00% within the BMI groups. The prevalence of metabolic syndrome and BMI-metabolic-risk sub-phenotypes in this young adult Nigerian population is high. These findings underscore the need for an urgent public health action in order to forestall the possibility of a looming public health crisis.

A multidisciplinary approach to medicine is something that is becoming more popular and widespread today. The combination of multiple health care disciplines can be extremely beneficial to progress an individual’s healing at a more rapid pace than relying only on one area for assistance. Amongst the most popular medical disciplines today, ancient ayurvedic medicine is most popular that many patients are seeking to relieve their ailments not only in India but also in other parts of the world. On the other hand, dental implants are the modern treatment modality that is popular for management of edentulism. However, majority of the elderly patients receiving dental implants are systemically associated with bone disorders that affect bone healing which in turn interferes with osseointegration. Here this review focuses on various pharmacological agents (including ayurvedic) for management of osteoporosis. For the review, data was collected from recently published articles (2001-2018) and multidisciplinary treatment options available for osteoporosis are summarized and reviewed.

Extraskeletal myxoid chondrosarcoma is a rare malignant soft tissue tumor of uncertain differentiation, constituting less than 3% of all soft tissue sarcomas. This low grade malignant tumor with high recurrence and metastatic potential, occurs predominantly in proximal extremities and limb girdles of males with thigh being the most common location. Here we present a case in a 35 years old female, who presented with pain and large swelling of around 8cm in lateral side of left leg since five months. Wide local excision of the tumor was done and on histopathology a diagnosis of Extraskeletal myxoid chondrosarcoma was rendered. On Immunohistochemistry the tumor cells showed reactivity for Vimentin, S100 and NSE and was non-reactive for CK. Thus our histopathological diagnosis was reconfirmed by immunohistochemistry. Despite high incidence of local recurrence and metastasis, ESMC has an excellent overall survival rate.
