Association between Polycystic Ovary Syndrome (PCOS) and Pregnancy Complications

Background: Polycystic ovarian syndrome (PCOS) pregnant women appear to be more vulnerable to poor maternal and neonatal outcomes. The purpose of this study was to assess the outcome of pregnancies in women with PCOS. Methord: This descriptive study was conducted at a tertiary care hospital Jashore Medical College hospital, Jashore Bangladesh, on 115 pregnant PCOS patients. SPSS version 20 was used for data analysis on the recent pregnancy, PCOS, and maternal and perinatal outcomes. Parity and others perinatal and maternal outcomes were examined as categorical variables. Result: The average age was 25.07 years, 82% had a history of primary infertility, and 66% had a high BMI. The percentage of pregnant women with hypertensive problems was 18.26%, PROM was 33.91%, and the low APGAR score at five minutes was 12.17%. gestational diabetes (23.47%), miscarriage (2.1%), preterm delivery (13.91%), caesarean delivery (37.39%), low birth weight babies (2.6%), macrosomia (0.7%), PPROM (19.52%), perinatal mortality (1.73%) and NICU admission (21.74%). Conclusion: Pregnant women with PCOS were identified with either similar or lower rates of GDM, miscarriage, preterm delivery, meconium-stained liquor, caesarean delivery, small for gestational age/IUGR, macrosomia, PPROM, perinatal mortality, NICU admission, and congenital anomalies, but higher rates of hypertensive conditions of pregnancy, PROM, low birth weight babies, and low five-minute APGAR scores.

Heart failure (HF) is a clinical condition that requires comprehensive medical care, consisting of supervision by a cardiologist, primary care physician (PCP), nurse practitioner and nutritionist. A physiotherapist, pharmacologist play a no less important role, and psychological support for both the patient himself and his family can be equally helpful, which can positively influence the treatment process. A significant problem in the care of patients with HF are repeated hospitalizations, which can result from the progression of the disease itself, but also from suboptimal therapy, comorbidities or noncompliance. As each hospitalization worsens the patient's prognosis, but just as importantly, generates high costs, outpatient care options involving different groups of medical personnel are being sought to improve the prognosis and quality of life of HF patients, but also reduce the cost of care. Currently, 90% of the budget for HF care is consumed by hospitalizations. This article aims to show how the competencies and tasks of a nurse can be used in the care and education of a patient with HF. The role of the nurse in the outpatient care of patients with HF is emphasized by the guidelines of the European Society of Cardiology, resulting in the creation of the Education and Certification Program for Heart Failure Nurses in Poland under the auspices of the Polish Cardiac Society, the Heart Failure Association, the Supreme Chamber of Nurses and Midwives.

Legal Reconstruction of Land Rights Allocation for Coastal Land Based on Justice Values

This research analyzes the weaknesses of the Land Rights Allocation for Coastal Communities and finds a legal reconstruction of the Land Rights Allocation for Coastal Communities based on justice values in Indonesia in a constructivism paradigm where the type of research method used is normative juridical and the specifications of this research have a prescriptive analytical nature with the approach used by the author being a statutory approach. The research results found that the Weaknesses can be seen from the fact that coastal communities have been given land rights for generations based on Minister of Agrarian Affairs and Spatial Planning/BPN Regulation Number 17 of 2016. Regulations on granting land rights to coastal communities have been implemented, but are not yet optimal, and the legal certainty has not yet been explained in detail the definition of the people who can be given Land Rights Land (indigenous communities/local communities/traditional communities), no clear boundaries, and no hereditary criteria. From a legal conceptual perspective, there will be confusion if a land-over-water settlement is given a clear certificate of ownership issued by the Land Office but the object to which the Certificate of Ownership of Land is entitled is actually on water, there needs to be an appropriate policy formulation to address this issue. so that legal certainty for people living in settlements on water can be realized. Therefore, the Regulations related to granting land rights to communities in coastal areas need to be harmonized and reviewed regarding the substance of the purpose and function of the certificate itself so that it can guarantee legal certainty and legal protection for holders of land rights certificates to improve community welfare in a broad sense.

Legal Reconstruction of Government's Political Regulation in the Election of Regional Heads Based on Pancasila Justice Values

This research analyzes the weaknesses of the government's political and legal regulations in the current implementation of regional head elections and finds a legal reconstruction of the government's legal politics in regional head elections based on Pancasila justice values in Indonesia. In a constructivism paradigm where the type of research method used is normative juridical and the specifications of this research have a prescriptive analytical nature with the approach used by the author being a statutory approach. The research results found that the weakness of the government's political and legal regulations regarding regional elections, the substance of which regulates the resolution of election crimes; Requirements as Candidates for Governor and Candidates for Deputy Governor, Candidates for Regent and Candidates for Deputy Regent, as well as Candidates for Mayor and Candidates for Deputy Mayor; Prohibition in campaigning; Criminal Sanctions Regional head elections; Simultaneous regional head elections; Community participation in organizing elections and the existence of a special electoral judicial body. Therefore, the reconstruction of the government's political and legal regulations for regional head elections as a whole based on the values of Pancasila justice in its formation can be done by synchronizing vertically with the 1945 Constitution and horizontally with the national election law starting from the planning, preparation, discussion, stipulation, and promulgation stages with adhere to principles in the formation of laws that Pancasila guides as the source of all sources of law in Indonesia by reconstructing the provisions governing the requirements for regional head candidates, candidate registration, voter data collection, campaigns, community participation, violations of the code of ethics, administration, dispute resolution, election crimes, state administration disputes, disputes over election results, formulation of criminal sanctions, implementation of simultaneous regional elections, as well as the formation of a tribunal for election crimes.

Revitalization of Islamic Education at Madrasah NWDI Lombok: Reviving the Heritage of National Heroes during the Colonial Era

This study aims to explore the efforts of revitalizing Islamic education at Madrasah NWDI in Lombok, Indonesia, with a focus on how Madrasah NWDI plays a role in preserving and reviving the national hero heritage of Indonesia during the colonial period. The primary issue addressed is how Madrasah NWDI has implemented the values of national heroes and local culture in its Islamic education, as well as how this relates to the challenges of contemporary education. The research employs historical research, document analysis, and interviews to gather relevant data. The findings reveal that Madrasah NWDI has played a significant role in upholding national and religious values and the spirit of national heroes. Education in this madrasah promotes character, morality, and leadership, which are relevant to the development of Islamic education today. This study makes a significant contribution to understanding the importance of Islamic education in preserving and rekindling the national heritage during the colonial era and its relevance in the context of present-day Islamic education. The conclusion underscores the pivotal role of Madrasah NWDI in inheriting and revitalizing the values of Indonesian national heroes. This research reinforces the understanding that Islamic education can serve as an agent of change that fosters national and religious spirit amidst ongoing societal transformations.

Stripe rust, caused by Puccinia striiformis f. sp. tritici, is a devastating disease of wheat in Ethiopia and the globe. The disease was dynamic and quite complex with the host (variety and plant spp), the environment, and with the pathogen genetic nature. The genetic alternation or change of a pathogen could affect the resistant population in the area. New race identification helps preparedness for the needs a rise to reduce the possible losses due to the problem. The race analysis dynamics identified three yellow rust races; namely, PstS11, PstS16 and ME2018 and one other unknown new race were the major in recent years but in the 2012-2016 Psts1 and PSTS2 were the dominant once. The new race ME2018 race mixture was increased the risk of stripe race epidemics in the area because it is Yr10-virulence race. So regular monitoring, for early detection and identification of new races was crucial.

Igniting Urbanization and Development of Mamfe Town through Road Infrastructure: Case of the Bamenda-Mamfe Highway

Road infrastructure is set to be a catalyst to urbanization and development in Sub-Saharan African towns. This article hypothesises that the Bamenda-Mamfe highway has contributed to the urban growth and development of Mamfe. The paper reveals that spatial growth, socio-economic development and trade of Mamfe town have been triggered by the highway. A multi-spectral Landsat satellite imagery of 1986 and 2020 were used to determine the spatial expansion of the town over a period of 34 years. Some 110 questionnaires were administered to households of 7 neighbourhoods in Mamfe to determine the outcome of expansion. Field observation and interview sessions were done to have information on the socio-economic development of the town. An in-depth secondary data collection was done to gather information on the rate of population growth in Mamfe. The data was processed to generate maps through the ArcGIS 10.4 and MapInfo soft wares and tables through SPSS 17 for results and analyses. Results from findings propound a rapid increase in the spatial expansion of Mamfe instigated by the construction of road as the urban space increased from 21% in 1986 to 59% in 2020. This was accompanied by a population upsurge of 119,000 inhabitants from 18,000 in 1987 to 137,000 inhabitants in 2020. There has been a remarkable increase in trade and commerce influenced by the construction of the road as people and goods can move in and out of the town with ease. The socio-economic development of Mamfe has as well improved with the presence of commercial centres, schools and health units. The development of Mamfe is however faced with planning challenges and urban ills which might escalate if not controlled. This paper recommends that stringent majors are taken to implement planning and proper security in Mamfe with the hope to render the town sustainable for living.

Associations among Qualitative Morphological Traits in Indigenous Camels of Ethiopia: A Multiple Correspondence Analysis

Camels play a crucial role in Ethiopia's livestock production and livelihoods, yet research on their phenotypic diversity is limited. This hinders the development of breeding programs and conservation strategies. This study aimed to address this gap by analyzing qualitative morphological traits in Ethiopian camel populations using multiple correspondence analysis (MCA), and to identify significant traits for potential use in breeding and conservation programs. Data on seven qualitative morphological traits were collected from 300 mature camels (150 from each district). Significant differences were found between districts for facial traits, specifically nose shape (p = 0.0153) and face profile (p = 0.0133). Other traits, including coat colour, hair type, ear orientation, and lip shape, did not show significant differences. MCA identified two main dimensions, explaining 26% of the total variance. The first dimension was associated with facial traits, while the second dimension was associated with coat colour and hair type. This study provides valuable insights into the morphological diversity of Ethiopian camel populations. The findings suggest that facial traits may be useful for differentiating camel populations and could be considered in breeding and conservation programs. Further research is needed to explore the genetic basis of these morphological differences.

Evolution and Prognosis of Digestif Fistulas in Crohn's Disease (CD) Other Than Ano-Perineal Manifestations (APM)

Background and Objectives: Crohn's disease (CD) is a chronic inflammatory disease of the intestine. It can evaluate by the occurrence of intestinal complications in particular fistulas whose management can be medical or surgical. The objective of our work is to specify the evolutionary modalities, as well as the predictive factors of bad prognosis. Patients and Methods: this is a monocentric, descriptive and analytical retrospective study conducted in a hepato gastroenterology department including patients with fistulizing Crohn's disease, outside ano perineal fistulas from 1990 to 2023. Statistical analysis was performed using SPSS22.0 software. Results: Of a total of 960 patients, 38 had fistulizing CD. 65% of our patients were classified as A3 according to the Montreal classification, and 34% as A2. They were 24 women with a sex ratio of 1.71 F/H. The type of fistula was Greco-Greatic in 4 patients (10.5%), Greco-Colic in 5 patients (13%), Entero-Cutaneous in 12 patients (31.5%), Gastro-Colic in 3 patients (7.9%), Interdigestive and Entero-Cutaneous in 10 patients (26%), Interdigestive and Greco-Vesical in 2 patients. All our patients had benefited from a bi-antibiotic therapy, 33 had benefited from surgical resection. Postoperative treatments were based on therapeutic abstention in 5 patients (13.1%), immunosuppressive treatment in 18 patients (47%), anti TNF treatment in 7 patients (18%). The endoscopic recurrence rate 6 months after treatment was 32.5%. In univariate analysis, the factors influencing the evolution of fistulizing crohn's disease were the number of fistulas and age with p values lower than 0.05 (respectively 0.022 and 0.009). Conclusion: Digestive fistulas represent a complication during CD, which pose a problem of medical-surgical management and have an impact on the quality of life of the patients. Our study showed that age (p=0.022) and the number of fistulas (p=0.009) represent the factors influencing the natural history of fistulizing CD.

Natural products are valuable source of pharmacological substances, and traditional herbal medicine is the source of many modern drugs used in pharmacotherapy. Many plant species belonging to the Asteraceae and Lamiaceae families are used as medicines for various ailments. The Satureja species has analgesic, antibacterial, antiviral, antioxidant, antiproliferative, anti-inflammatory, and vasodilatory qualities. Many kinds of modern drugs, such as aspirin, are derived indirectly from medicinal herbs. Many food crops have medicinal qualities, such as garlic. Among the many naturally occurring phenolic compounds found in herbal plants is ferulic acid (FA). The most harmful environmental stress that is thought to reduce agricultural productivity more than any other is drought. Drought stress, for example, inhibits plant growth because water is necessary for cell turgor, which is the force that a confined liquid applies to cell walls to induce cells to enlarge. All stages of a plant's life cycle, including seed germination, seedling development, vegetative growth, and blooming, are significantly impacted by salinity. Energy transfers, plant maturity, fruiting, and seed formation are all significantly influenced by phosphorus.

Effect of Weight Training on Increasing 100 Meter Running Speed in Sprinter Athletes Based on Gender

The aim of this study was to investigate the effect of weight training on the performance of 100meter spinner athletes based on gender. This research is one group pretest – posttest. Samples on athletes were carried out voluntarily provided that they were not injured and were not taking part in a competition period. The sample consisted of 10 male sprinters (mean: height 170.2±0.6 cm, weight 64.2±2.7, body mass index (BMI) 21.27, training experience 5.6±7.8 years) and 10 female sprinters (mean: height 168.7±7.2, weight 53.7±0.6, body mass index (BMI) 18.14, training experience 5.2±14 years) with age 17 – 22 years. The test for pretest–posttest is 100 meters run, while determining the load volume uses the 1RM Brzycki formula. This training program is carried out for 6 weeks (3 times a week), training volume 40% - 60% with 3 sets x 8 repetitions, types of weight training are bench press, deadlift, leg press, half squat, double leg bounds. The results of the t test, this research shows that weight training has an effect on increasing the 100 meters sprint of male athletes by 0.000 < 0.05 with an increase in time of 0.33 seconds. Then weight training has an effect on increasing the 100 meters sprint of male athletes by 0.000 < 0.05 with an increase in time of 0.20 seconds. The recommendation of this research is to train at the general preparation stage, because this weight training using 2 supports on the right and left sides of the hands and feet.

This study explores the transformative implementation of a Concept-Based Approach (CBA) in nursing education at the University of Calgary in Qatar (UCQ). Focusing on the Family in Transition Nursing course, the research examines how CBA, characterized by its emphasis on critical concepts and practical exemplars, bridges theoretical knowledge with clinical practice. Utilizing digital platforms like Desire to Learn (D2L) and tools like Padlet, the approach enhances student engagement and learning through real-world applications. The study highlights the shift from traditional teaching methods to CBA, aiming to produce skilled nursing graduates capable of managing the complexities of the Qatari healthcare system. It investigates the effectiveness of CBA in deepening learning, its application in real-life scenarios, and its impact on teaching practices. Results underscore the importance of content quality, concept-focused learning materials, and real-life case studies in nursing education. The research concludes with the CBA's significant role in UCQ's nursing program and addresses future research directions to further understand and develop concept-based teaching strategies in nursing and beyond.

Bilateral Subcapsular Orchidectomy as Surgical Castration: A Reasonable Aesthetic Alternative to Bilateral Total Orchidectomy in Patients with Metastatic Hormone-sensitive Prostate Cancer

Background: The study highlights the historical significance of bilateral orchidectomy as the traditional 'gold standard' for surgical androgen deprivation in treating advanced prostate cancer. The study presented aims to compare total orchidectomy and subcapsular orchidectomy, considering factors such as androgen ablation, disease progression control, and patient satisfaction. Objective: This study aims to compare the effectiveness and patient satisfaction of bilateral subcapsular orchidectomy and bilateral total orchidectomy treatment in managing hormone-sensitive metastatic prostate cancer. Methods and materials: In this prospective study of 18 months at North East Medical College Hospital, 40 participants with Metastatic Carcinoma of Prostate underwent bilateral orchidectomy, among them 20 subcapsular and 20 total orchidectomy. The research focused on evaluating the impact of these treatments on serum testosterone levels, serum PSA levels, employing as outpatient procedures and a 3-month follow-up, utilizing a satisfaction scoring scale. Data, including age, Gleason Grade Group, PSA, and testosterone levels at diagnosis & at 3-month follow-up were recorded and analyzed using IBM SPSS-21 software. Result: The study compared Bilateral Subcapsular Orchidectomy and Bilateral Simple Orchidectomy for Hormone-Sensitive Metastatic Carcinoma of Prostate in 40 patients. The highest frequency was in the 71-75 age group (30%), with a mean age of 66.67 ± 2.21 years. Gleason grade group 3 dominated (40%), followed by 2 (25%). Pre-operative PSA levels were 31.14±1.27 ng/ml for Subcapsular and 35.21±1.70 ng/ml for Total Orchidectomy, decreasing post-operatively to 8.25±0.41 ng/ml and 7.32±0.80 ng/ml, respectively. Pre-operative testosterone levels were 513.21±3.01 ng/dl for Subcapsular and 498.40±2.10 ng/dl for Total Orchidectomy, decreasing post-operatively to 21.14±2.84 ng/dl and 16.90±1.08 ng/dl, respectively, with non-significant p-values. Surgery related Satisfaction scores in the Follow-Up phase were significantly higher for Subcapsular Orchidectomy, 2.91±0.31 comparing with Total Orchidectomy, 2.05±0.45. The results emphasize better patient satisfaction after subcapsular orchidectomy, while maintaining similar cancer control in the form of PSA and testosterone level changes. Conclusion: In conclusion, our study advocates for the reconsideration of bilateral subcapsular orchidectomy as a preferred method for surgical androgen ablation in metastatic prostatic carcinoma, offering comparable efficacy to traditional total orchidectomy approach while prioritizing patient satisfaction and psychological well-being. This suggests a potential paradigm shift in the landscape of cost-effective androgen deprivation therapy within the urological field.

In vitro and In vivo Assessment of the Effect of Okra Gum Solid Dispersion in Atorvastatin Solubility

Background: Atorvastatin is BCS class II drug; it is lipid-lowering medication. Okra gum (OKG), from the pods of Abelmoschus esculentus, is natural product contain polymers having advantages over synthetic ones as it is safe, chemically inert, nonirritant, biodegradable, and does not require toxicological studies. Aim: This study aimed to assess the effect of okra gum solid dispersion in atorvastatin solubility. Method: The gum was extracted by hot water extraction and the dry extract was evaluated for percentage practical yield, flow properties, pH values and FTIR spectroscopy. Then solid dispersions with different drug to polymer ratios were prepared from OKG, and hydroxy propyl methyl cellulose (HPMC) by solvent evaporation method. Saturation solubility was tested for the solid dispersions prepared, the physical mixtures and atorvastatin. Tablets were prepared from solid dispersions with the highest saturation solubility, then tablets were tested and evaluated. Finally, in vivo test was done using Swiss albino mice and data were analyzed using one way Anova test followed by T test. Results: The content percent of atorvastatin in the solid dispersion prepared were 99,9- 100.1%, the tablets showed satisfactory physicochemical properties as 1.29% RSD in tablet weight variation, 24 min disintegration time, 5.24±0.457 Hardness and OKGSD tablets showed sustained release manner and 87% of drug released in 6 hrs. Lipid profile results showed significant decrease in total cholesterol level with marked decrease in LDL when using OKGSD tablets. Conclusion: It was concluded that OKG is promising excipient that can be used in dosage forms formulation to enhance solubility of low soluble drugs.
