Hemophagocytic lymphohistiocytosis (HLH) is a clinical-biological entity characterized by a hyperinflammatory state following the deregulation of the cytotoxic immune response, causing histiocytic proliferation with significant hemophagocytic activity in the bone marrow as well as the massive release of inflammatory cytokines. The diagnosis is established by the association of nonspecific clinical and biological signs. The aim of this study was to describe the clinical and laboratory presentation to determine the underlying pathologies and clarify the evolution and the prognosis of adult patients with HLH in our healthcare setting. A retrospective study of patients with HLH was conducted in the Military Teaching Hospital, Rabat, between January 2015 and January 2017. We were able to identify eight patients. Six were men, and the average age at diagnosis was 58 years. Infectious agents were responsible in 3 cases, malignant lymphoma in 3 others, one case of systemic lupus erythematosus, and in the last case etiology remains unknown. Clinical characteristics such as fever (100%), splenomegaly (62,5%), and lymphadenopathy (37,5%) were observed. Laboratory values presented with panytopenia (62, 5%), thrombopenia and anemia (100%), increased ferritin (100%), lactate dehydrogenase (87, 5%), triglycerides (62, 5%), and decreased fibrinogen (50%). Bone marrow showed hemophagocytis in all cases. The prognosis was poor. Five patients died. Absence of specific signs makes early diagnosis difficult. The prognosis varies according to the series but remains very unfavorable. Studies are needed to assess prevalence and develop a severity score and therapeutic recommendations to improve HLH prognosis

Anxiety is a neuropathlogical condition which disturbs the normal routine life and CNS function. Serotonin is well known neurotransmitter responsible for pathogenesis of neurological diseases such as epilepsy, migraine, multiple sclerosis and Parkinson's disease &neuropsychiatric diseases like depression, anxiety and stress. Malfunctioning of 5-HT receptor, 5-HT transporter and mutation in gene code for its synthesis may contribute to abnormal function of serotonin. Nigella sativa is most common herb use as medicinal plant .In present investigation aqueous extract of Nigella sativa given to rats exerts anxiolytic and hyperserotonergic effect by increasing serotonin level in rat brain. Significant anixolytic effects were observed in open filed box and elevated plus maze. All these findings proved that Nigella sativa can be beneficial to treat neurological behavior defect which is due to hypoactivity of serotonergic neuron in brain

A Validated Reversed Phase HPLC Assay for the Determination of Dexamethasone in Human Plasma

A simple and precise reversed-phase high performance liquid chromatography (HPLC) assay for the determination of dexamethasone in human plasma was developed and validated. 1.0 ml plasma samples containing dexamethasone and lansoprazole as internal standard (IS) were deproteinized with 2 ml dichloromethane and 3 ml methyl tert-butyl ether. Separation was achieved on Atlantis dC-18 column with a mobile phase consisted of acetonitrile and water (40:60, v:v), and delivered at a flow rate of 1.0 ml/min. The eluent was monitored spectrophotometrically at 240 nm. No interference in blank plasma or of commonly used drugs was observed. The relationship between the concentration of dexamethasone in plasma and peak height ratio of dexamethasone to the IS was linear over the range of 0.15-8.0 μg/ml. Intra-day and inter-day coefficient of variation (CV) and bias were ≤11.1% and ≤9.5% and ≤9.3% and ≤5.6%, respectively. Mean extraction recovery of dexamethasone and IS was 100% and 97%, respectively. The method was applied to assess dexamethasone stability under various conditions encountered in the clinical laboratory. Dexamethasone stability in processed (24hrs at room temperature, 48hrs at -20°C) and unprocessed (24hrs at room temperature, 8 weeks at -20°C, or after 3 cycles of freeze and thaw) samples was ≥ 90% and ≥ 83%, respectively
Isolated laryngeal tuberculosis is rare and presents less than 1% of the manifestations of this disease, it poses a problem of differential diagnosis with other affections mainly tumors. The aim of this work is to emphasize the diagnostic difficulty of this entity and its management. We report the case of a 21-year-old patient hospitalized for progressive dysphonia. Endoscopy showed an ulcerous-budding process of the two vocal cords (which were mobile) extending to the laryngeal vestibule and vestibular bands. The histological examination of the biopsies performed was in favor of a caseo-follicular tuberculous laryngitis. An anti-bacillary treatment has been introduced which has allowed a total resolution of clinical, biological and endoscopic signs. La is healthy 2 years after the end of treatment.

The objective of the present study was to develop and optimize an oral pulsatile drug delivery system containing pantoprazole sodium to mimic the circadian rhythm of the peptic ulcer by releasing the drug with a distinct predetermined lag time. Six fast disintegration core tablets were prepared for preliminary trials using direct compression method. The tablets were evaluated for hardness, friability assay and dissolution study. The best formulation were selected for optimization to study the influence of Micro crystalline cellulose (MCC) and Sodium starch glycolate (SSG) using 32 full factorial design. The optimized formulations were selected for coating for pulsatile delivery. The results of the study indicate f3 formulation was suitable for scale up

The population structure, growth, mortality and exploitation status of Euthynnus alletteratus were examined in the Gulf of Guinea between January 2015 and December 2016. Monthly length frequency data of Euthynnus alletteratus were analyzed by FiSAT II software for estimation of population parameters which include asymptotic length (L∞), specific growth level (K) and recruitment pattern to evaluate the status of the stock. Asymptotic length was 100.8 cm, and specific growth level was 0.07 yr-1, value was too low, while the growth performance index was 2.85. Total mortality (Z) by length-converted catch curve was estimated at 0.82 yr-1, fishing mortality (F) 0.61yr-1 and natural mortality (M) 0.21 yr-1. The recruitment pattern was continuous all the year with two peaks of unequal importance: the secondary appears between March and April while the main is observed from July to August. The exploitation level (E) of Euthynnus alletteratus was 0.74 that on the maximum allowable limit of exploitation (Emax) which was 0.42. The exploitation level (E>0.50) indicates that the fishing pressure on Euthynnus alletteratus is very high. Thus the status of the stock in the Gulf of Guinea seems to be surexploited

HIV/AIDS is a serious challenge for the developing as well as the developed world. Globally, 35.0 million (33.2–37.2 million) people were living with HIV at the end of 2013. [1] Although many HIV‑infected individuals avoid risky behaviours, still substantial numbers of HIV‑infected persons, continue to engage in HIV transmission risk behaviours. To find out the Socio-Demographic, Behavioural characteristics and Clinical profile of People (Males more than 15 years) Living with HIV/AIDS (PLHA). An institutional based cross sectional study was carried out among People (male >15 years) living with HIV /AIDS (PLHA) attending Anti Retro Viral Therapy (ART) Centre of Calcutta School of Tropical Medicine (CSTM) for 12 months from May 2012 to April 2013. A predesigned and pretested schedule was used as study tool to collect required information. Maximum population (40.5%) belong to the age group 31-40 years followed by 24.1% in 41-50 years. Majority had coital debut at the age of 26-30 years (35.9%). First sexual partner among 41.7% was female commercial sex worker; 74.4% participants were not into monogamous relationship. More than half of study population (51.1%) were currently sexually active and 55.36 % of currently sexually active participant consistently used condom. Majority of the study population suffered from tuberculosis (28.2%) followed by candidiasis (24.5%). PLHA should be trained on safe sex strategy, correct and consistent use of condom among themselves & their partner as well as maintaining monogamous relationship. More involvement of Community Care centre to reinforce, care and counselling, support of PLHA against Opportunistic infection will reduce their poor health burden. IEC activities to promote value based lifestyle with reducing vulnerabilities among PLHA and creating a strong network among PLHA will prevent such harmful behaviour

Metastasis of unknown primary origin (MUPO) account for 2–3 % of all malignancies in Western countries and represent a heterogeneous, often aggressive, and clinically challenging group of tumors with early metastatic dissemination for which a standardized diagnostic workup initially fails to identify the site of origin at the time of diagnosis. In the following chapter, we review the possibilities and challenges of tissue-based conventional as well as immunohistochimical procedures to categorize this heterogeneous group of neoplasms. We describe the role of pathology in MUPO diagnosis as part of a multidisciplinary effort primarily involving oncologists, surgeons, radiologists, and pathologists with the ultimate goal to aid in clinical reasoning and decision making.

It is important for any health system to provide Nursing care with great quality and respond the client’s necessities. There are three dimension of nursing care which is used to assess patient’s satisfaction that are Nursing skill, Attention and Somatic Care. Purpose of study is assessing satisfaction of patient’s related quality of nursing care dimension in the government center of Lahore Pakistan. Quantitative descriptive cross sectional study design was used. Simple convenient sampling was used to collect the data from two different government hospitals. Sample size was 200 patients from selected hospital. The results of the study revealed that vast no of respondents (69.9%) were gratified with nursing care provided to them, while others out of 194 respondent, 44 participants are 60.9 % positively remarks that nurses are always cheerful towards the patient .They are mostly kind or soft hearted. 16.5% of 32 respondents are neutral about this question. And 22.7 % respondents 128/194 were having negative response for this statement.69.6% are satisfied and agree with the statement of this question because they thought that nursing is a noble profession and the nurses who works day and night for the well beings of the patient are admire able. Study results showed that mostly participants remained satisfied with care of nursing; consequently they recommended some development particularly with respect to the interpersonal relation. Additional research will be needed to study comprehensive socio cultural and environmental factor related to client gratification regarding nursing quality care

Carcinomatous Meningitis from Solid Tumors: A Seven Years’ Experience in Moroccan Patients

Carcinomatous meningitis from solid tumors has not been investigated in detail in the Moroccan population. Therefore, we reviewed our experience with this disease to determine its clinical features and treatment outcomes in Moroccan patients. We performed a retrospective review of all patients with solid cancers admitted to the Military Hospital Moulay Ismail in Meknes, Morocco with carcinomatous meningitis between January 2011 and December 2017. A total of 12 patients were included. 5 were males and 7 were females. The median age was 54 years old (39 – 77 years old). The most frequent primary tumor was breast cancer (50%) and lung cancer (34%). The median time from initial cancer diagnosis to the development of carcinomatous meningitis was 6.9 months (0 – 49 months). The most common symptoms were headache (67%), nausea and vomiting (42%), and vision changes (34%). The diagnosis of CM was established by both CSF cytology and MRI in 25% of patients, by CSF cytology alone in 17%, and by MRI alone in 58%. One of the 12 patients received supportive care only. Chemotherapy was the only treatment for 50% of patients, 17% received radiotherapy alone, and 25% received both chemotherapy and radiotherapy. The median survival from the time of carcinomatous meningitis diagnosis was 9 weeks (2 – 29 weeks). Our results confirm the continued poor prognosis of carcinomatous meningitis in Moroccan patients with solid tumors as well. Further clinical trials are needed to improve therapeutic strategies including prophylactic approaches
