Obstetric Outcomes of Pregnant Women with Eclampsia

Introduction: Over half a million women die each year from pregnancy-related causes, 99% in low and middle-income countries. In many low-income countries, complications of pregnancy and childbirth are the leading cause of death amongst women of reproductive years. Overall, 10% to 15% of direct maternal deaths are associated with pre-eclampsia and eclampsia, where maternal mortality is high; most deaths are attributable to eclampsia, rather than pre- eclampsia. This study aimed to analyze the obstetric outcomes of pregnant women with eclampsia. Methods: This prospective study was conducted at the Department of Obstetrics & Gynecology in Uttara Adhunik Medical College & Hospital, Dhaka, Bangladesh for 6 months; from June 2019- November 2019. A total of 50 subjects were included in this study. Informed written consent was taken from the study subjects. Data was collected using a pre-formed data sheet. Data processing and analysis were done by using SPSS version 17. All patients underwent necessary investigations. All information was kept confidential and used only for this study purpose. The ethical Clearance Certificate was obtained from Bangladesh Medical College. Result: In this study, most of the patients (30, 60.0%) belong to the age group of 18- 25 years, followed by 12 patients (24.0%) were from the>35 years age group and the rest 08 (16.0%) patients belonged to 26-35 years, age group. Most of the patients (35, 70.0%) were nulliparous followed by multiparous (15, 30.0%). Maximum respondents (26, 52.0%) developed seizures at the antepartum stage, followed by 18 patients (36.0%) at the intrapartum stage and the rest 03 (6.0%) patients developed seizures at the postpartum stage. Concerning complications, maximum patients (08, 16.0%) suffered from hemorrhage, followed by anemia (7, 14.0%), abruptio placenta (6, 12.0%), sepsis (5, 10.0%), obstructed labor (4, 8.0%). Regarding maternal and fetal outcomes in eclampsia, this study revealed satisfactory outcomes in 37 (74.0%) patients with prompt actions taken during the condition. However, perinatal death occurred in 2 (4.0%) cases, and maternal death occurred in 1 (2.0%) case. Preterm delivery was done on 10 (20.0%) patients. Conclusion: Obstetric outcome was somewhat satisfactory in this study due to immediate diagnosis and actions taken during eclampsia, however, perinatal death occurred in 2 (4.0%) cases and maternal death occurred in 1 (2.0%) case and 10 (20.0%) cases underwent preterm delivery.

Pregnancy Outcomes in Patients with Placenta Previa Due to Site of Placentation

Background: The aim of this research is to evaluate pregnancy outcomes in patients with placenta previa due to the site of placentation. Method: This retrospective study included 105 cases conducted in the OBG department of Al AIN hospital for three years (January 2015 to October 2018). All cases of placenta previa admitted during this period were included in the study. Case records were obtained from the medical record section and carefully analyzed to find out the incidence, various types of placenta previa, its clinical presentation, and its outcome in relation to mode of delivery, birth weight, and maternal and perinatal morbidity. Results: Placental attachment site influenced the outcome of pregnancy. Placental attachment to the anterior wall was associated with shorter gestational age, low birth weight, low Apgar score, higher prenatal bleeding rate, increased postpartum hemorrhage, longer duration of hospitalization, higher blood transfusion and higher hysterectomy rates compared to cases with lateral/posterior wall placenta. Placental attachment at the incision site of previous cesarean section significantly increased the incidence of complete placenta previa compared with placental attachment at a site without incision, but did not significantly influence pregnancy outcomes. Placental attachment to the anterior wall was an independent risk factor for postpartum hemorrhage in patients with placenta previa. Conclusion: The site of placental attachment in patients with placenta previa influences the pregnancy outcome. When the placenta is located on the anterior wall, clinicians should pay attention to the adverse pregnancy outcomes and the possibility of massive postpartum hemorrhage.

Cervical Cancer during Pregnancy: Case Report and Literature Review

Cervical cancer coexisting with pregnancy is a rare occurrence with an incidence estimated between 1 and 2/10,000 according to studies. The diagnosis presents a challenge due to cervical changes observed during early pregnancy, and treatment must balance the imperative to manage the mother's cancer while also considering the fetus. This association poses four major problems: the difficulty of diagnosis, the prognosis of the disease, the timing of surgical treatment, and the effect on the pregnancy and mode of delivery. This study presents a series of five cases of cervical cancer discovered during pregnancy between 2010 and 2013. The therapeutic management is similar to that of non-pregnant patients, although some adaptations are necessary due to the gravid state, and pregnancy does not appear to alter the prognosis of the cancer.

We present a case of a 26-year-old female at 37 weeks of gestation with concomitant pituitary apoplexy and acute fatty liver complicated with disseminated intravascular coagulation, leading to foetus death and hemorrhagic shock secondary to a massive uterine haemorrhage which was successfully managed in Intensive Care Unit. To the best of our knowledge, this is the fourth case reported in the literature.

Background of the study: Erectile dysfunction may be responsible for a reasonable amount of male infertility. Erectile dysfunction can lead to male infertility. Male infertility is due to any problem in the male reproductive system including decreased sexual desire, barrenness, premature ejaculation, etc. Method: 20 male Wistar rats and 5 female Wistar rats were used for this study. Male animals were grouped into control, Viagra, high-dose Bay leaves, and low-dose Bay leaves. The extract was administered for 21 days. The animals were acclimatized for one week. Extracts were administered for three weeks. A sexual behavioural study was carried out on the animals in the dark for three days. 24 hours after the last day of the sexual behavioural study, the animals were sacrificed and the testes were harvested and sent for histological studies; semen was collected for semen analysis. Results: Data were expressed as mean ± SEM. The mean difference between the tested groups and the control was tested using a one-way ANOVA. Values were considered statistically significant when P is ≤ 0.05. Mount frequency increased significantly in high-dose groups of both premature plant and bay leaves treated animals. Conclusion: This implies that the plant extract used for the present study increases sexual function (libido and potency).
