Perceptions and Expectations of Providers on Maternity Rights of Parturients

Introduction: This study aimed to understand the perception of providers of the Mama Mosalisi health and maternity center perceive the rights of parturients and to explore their expectations. Method: We conducted a descriptive qualitative exploratory phenomenological study, the data collection was done through individual face-to-face interviews with 10 providers of the Maman Mosalisi Health Center (nurses, laboratory technicians, doctors, midwives), worker in the maternity ward. Results: Providers generally have a positive perception, characterized by the recognition of the rights of parturients. They affirm that it is important to respect the rights of the parturients because this respect makes it possible to avoid the bad course of the childbirth; to gain the confidence of parturients; to increase the clientele and the receipts in the structure; leads to conscientious work; and contributes to the reduction of mortality. In their experiences, service providers encounter several difficulties in the exercise of their profession; but they also recognize their failure to respect the rights of parturients, they feel this in the form of guilt. Finally, they expect managers to improve working conditions in their structure, provide for self-assessment sessions, that the State take charge of them in everything, that the organizations involved strengthen their capacities through continuous training, that researchers sensitize the public in general and parturients in particular on their rights which they are unaware of, in order to be able to claim them and reframe the practices of providers. Conclusion: These results plead in favor of concerted interventions between political and health decision-makers, the partners involved and the providers for the promotion of respectful maternity care in order to contribute to the reduction of maternal and infant mortality.

Background: The Education system has taken a 180-degree paradigm shift since Covid-19 wrapped up the prevailing global system. Now educational system used made electronic communication devices the prerequisite. Not only were the students, parents, and teachers dragged out of their comfort zone to learn and stretch their expertise in the digitalization of education and work. The endeavor has brought us to set standards and meet the needs of new standers. This study aimed to discover the suggestions of students and faculty to deal with the resentment that is being experienced with this online facade. Methodology: Cross-sectional study from March to May 2020, total participants was 523 including male and female medical students and faculty. Ethical approval was obtained from Ethical committee CMH Lahore Medical College and Institute of Dentistry (CMH LMC & IOD). Verbal informed consent was obtained. Self-administered questionnaires were given to the participants. Results: E-Learning opportunities need to be taught explicitly. Modern tools are the need of time. The faculty needs to understand student psychology as well as the application of modern tools and gadgets. Conclusions: E-learning posed a technological divide among the faculty members and students. The pandemic allowed us an opportunity to explore the realms of the digital world and expand our horizons to challenge the already existing medical education system, curriculum, and mode of its execution.

Efficacy of Ferrous Fumarate in Comparison to Carbonyl Iron in the Treatment of Iron Deficiency Anaemia during Pregnancy in Rajshahi, Bangladesh

Introduction: Iron deficiency represents a spectrum ranging from iron depletion to iron deficiency anaemia. In iron depletion, the amount of stored iron is reduced but the amount of transport and functional iron may not be affected. Iron deficiency is a major problem worldwide especially in women of reproductive age. Objectives: This study was designed to compare efficacy of two commonly used oral iron preparations among anemic pregnant women in private chamber of gynecologists and outdoor of Rajshahi Medical college hospital, Rajshahi, Bangladesh. Methods: We conducted analysis of data collected from Pregnant women between 18 to 40 years of age, >8 weeks of gestation, having iron deficiency anaemia (serum hemoglobin levels <10 gm/dl or serum ferritin <30μg/l). The patients were divided into 2 groups (n=30) each and treated with Ferrous fumarate and carbonyl iron respectively. Hemoglobin gm/dl and serum Ferritin ng/dl were recorded after the interval of 3 months from baseline. Data was analyzed using SPSS 16. Efficacy variables between groups calculated using Chi square and T- test. Results: We observed that patients treated with Ferrous Fumarate (FF) showed significant rise in Hb 0.69(±0.83) which was greater than that of Carbonyl iron (CI) group 0.07(±1.11) g/dl. Mean (± SD) rise of serum ferritin was significantly higher in FF group (55.45 ±34.66) as compared to CI group (19.88 ±15.33) (t=5.14, df =58, P<0.001). Conclusion: It can be concluded that, ferrous fumarate still can be considered better effective medication than Carbonyl iron for the treatment of Iron deficiency anemia in pregnancy.

International news plays a crucial role in shaping the image of a country [1]. In this article, seven reports on the new coronavirus in the Chinese and American news media, CGTN and USA Today are used as the research materials. Through the comparative analysis of the Attitude and Graduation system of the Appraisal theory, we find the similarities and differences in the discourse between China and the United States for further analysis and further analyze the relationship between discourse expression and national image. At last, we put forward corresponding suggestions to international reports; promote the spread of Chinese power and the positive construction of Chinese international image.

The quality of teaching shapes the future of students and prepares them to be a responsible citizen. Therefore, all efforts to improve the quality of education are dependent on the instructional competence of teachers and their capacity to undertake the various important roles and functions. These endorses the need for teachers to embrace concrete mechanisms of enhancing their instructional competences to constantly innovate and adapt to dynamic student needs for their Competency-based mastery. In this regard interrelation of instructional competency with teachers’ performance appraisal and development is of interest because of its relevance and vitality in endorsing a pedagogical culture which resonates with education quality despite the teacher’s negative perception. However, there is a dearth of literature explaining whether the two major approaches to teacher evaluation namely value-added measures and standards-based evaluations affect their instructional competence providing a gap for the current review. From the review value-added measures and standards-based evaluations significantly affects instructional competence despite their various limitations. Value added measures can enhance the instructional competence of the teacher courtesy of the synergy amongst collaborative learning communities can assess the evidence of student learning based on the test scores for effective teaching.The headteachers within the policy frameworks should cooperate with the teachers in determination of standards which should be realistic, viable and reliable in addressing instructional competence. However, performance appraisal, might have catastrophic consequences if it is carried out without clear objectives.

Postpartum depression is one of the health problems that may occur without being recognized which may lead to a serious adverse effect for mother and her child. Postpartum depression remain underestimated and unrecognized as many women are discharged without being detected as having PPD or even uneducated in order to seek for help when needed. Postpartum depression has a negative impact on both mother and the child wellbeing. Early detection is essential for mothers’ and infants’ safety. Postpartum depression is defined as “a disorder that is often unrecognized and undertreated”. The aim of this paper is to go through an analysis for postpartum depression detection and get a clear picture about the definition, attributes, antecedents, consequences and its empirical References. Method: a review of the literature was conducted. Articles from 2013 to 2020 were included. Walker and Avant framework was used for concept analysis. Results: the search have found four attributes: time, risk factors, symptoms, and outcome. Also, the search have found the following antecedents: knowledge, screening and care. The consequences are access to mental health care and drawing the treatment line process. Conclusion: this analysis will help in discovering the suitable methods for detecting the postpartum depression to achieve high quality care for mothers and infants.

Background: Sick sinus syndrome (SSS), also known as sinus node dysfunction (SND), is a disorder of the sinoatrial (SA) node caused by impaired pacemaker function and impulse transmission producing a constellation of abnormal rhythms. Sick Sinus Syndrome is a highly relevant clinical entity, being responsible for the implantation of the majority of electronic pacemakers worldwide. Case Summary: An 80-year-old patient with a recent diagnosis of atrial fibrillation after a syncopal attack, presented to our clinic complaining of new onset lightheadedness. Following a pre-syncopal attack in our clinic, extensive analysis was performed and showed no underlying causes for the light headedness. A 24-hour Holter monitoring was performed and demonstrated signs of sick sinus syndrome with unusually prolonged sinus pauses without an escape rhythm. Urgent correction with permanent dual chamber pacemaker was performed with an uneventful postoperative recovery. Discussion: In sick sinus syndrome, cessation of sinus rhythm (sinus arrest) for short intervals without an escape rhythm, or longer periods of pause with replacement of sinus rhythm by an atrial or junctional rhythm are commonly seen. However, recurrent prolonged ventricular asystolic episodes are infrequently encountered and an extremely rare cause of syncope. It is known that triggers like anaesthesia, certain drugs or epilepsy can unmask sinus node dysfunction, which then manifests as significant atrial dysrhythmias and “rarely” asystole but long asystolic episodes of such duration were never reported in the absence of any trigger in patients with sick sinus syndrome. Permanent pacemaker placement is the recommended treatment for sick sinus syndrome.

Introduction and Research Problem: The study aims to illustrate the role of language in depicting and representing identity in relation to different social classes. In the African American sitcom of the study at hand, different social classes of the same black community/race are represented in the portrayal of different characters, in term of their language use as well as other behavioral aspects. However, since social classes, apart from being socially structured, are the root source of so many different stereotypes. Materials and Methods: A sample is taken from the African American sitcome: The Fresh Prince of Bel Air. The reserchers has watched the whole six seasons periorly. However, for the matter of the study at hand, 15 episodes are selected specifically form the twenty-five episode long first season which specifically chosen because it includes the introduction and fundamental characteristics of each characters’ personality together with the role they play and the function they perform. Summary of Results: The portrayals of the characteristics of each character weight heavily on common stereotypes related to the social classes that characters belong to. Conclusion and Recommendations: Upon analyzing the behaviors of the main characters of the African American Sitcom, we come to the conclusion that, characters’ portrayals match the notions of such stereotypes. Yet, through the different acts of the characters, we realized, that different misconceptions of stereotyped images were revealed to be misconceived.

Plant based traditional health care is one of the ancient remedies used to prevent and treat different health related disorders. Due to increasing cost of medicine in the modern era, people are now moving towards the utilization of ancient ethno medicinal plants based remedies to prevent and treat diseases as well as to maintain their health. Curcuma longa, commonly known as turmeric has been used since ancient times as ethno medicinal plant due to its pharmacological and therapeutic potential. The rhizome of this plant is commonly used to prevent the lifestyle related disorders. Its biologically active components can also be extracted and utilized directly to enhance the efficacy. Purpose of this review is to highlight the importance of turmeric as it contains various biologically active components that are beneficial in prevention and treatment of various health related disorders. Turmeric has been demonstrated to exhibit anti-cancer, immunostimulant, skin protection, ulcer treating, anti-inflammatory, anti-malarial, anti-bacterial, anti-fungal, anti-viral, anti-parasitic, anti-hyperglycemic, anti-oxidant, anti-hyperlipidemic, hepatoprotective, renal protection and hematological parameters maintenance properties. There is no evidence of adverse effects of turmeric in literature. Only the people who are allergic to it can have side effects otherwise it is almost stomach friendly due to which it can be used for treatment of various health related disorders.

A study was conducted to identify the challenges and opportunities faced by female employees in education institutions and how well the female employees can manage their professional life and personal life. Research is steered at one of the University in Sultanate of Oman. It is a descriptive study conducted to find the opinions of female employees towards work life balance at University in Oman. The population of the proposed research include all female teaching staff of the University (population is 83) by including all the departments (BSD, ELC, Engineering and IT). The sample size is 68 with (95% confidence level). To know the intentions and opinions of female employees a structured questionnaire is administered for collecting primary data. Stratified proportionate convenient sampling method is used to collect data from female teaching employees of the University. Parametric test ANOVA is used for analysis. A one-way ANOVA was conducted to compare the opinions of female employees towards work environment. The employees’ targets that create tension shown a statistically significant difference among four departments. The employees’ responsibility at work create stress shown a statistically significant difference among four departments. The employees not able to spare time for research activities shown a statistically significant difference was found among four departments. It is observed that there is a difference of opinion among female employees of the university towards work environment. Similarly, there is a difference of opinion among female employees of the university towards home environment (family) and regarding work life balance among four departments. Post-Hoc test was conducted to know the actual difference of their opinions with reference to other departments and found that IT and engineering has a difference of opinion regarding work life balance. Hence, we can say that some female employees are not comfortable with work life balance at the University.

Impact of Inflammatory Markers in Predicting Outcomes of SARS-CoV-2 Patients: The why’s and how’s of Diabetic Hypertensive Patients

Background: Hypertension and Diabetes Mellitus, the most frequent co-morbidities in SARS-Cov-2 patients are considered as risk factors for disease severity and mortality. And the role of inflammatory biomarkers on these patients is still under evaluation. Methods: Retrospective data was collected study in 3 independent groups each with 24 patients: diabetes, hypertension and with concomitant diabetes and hypertension respectively of critically ill SARS-CoV-2 patients admitted in the CCM. The data of age, gender, diabetic and hypertensive history, inflammatory marker, duration of stay was obtained from electronic patient data repository of the hospital and compared with Mortality. Results: The mean CRP levels in diabetes was 78.81 ± 16.8, in hypertension 82.23 ± 13.98 and in group with both the co morbidities was 79.05 ± 16.38. Serum ferritin levels were high in hypertensive group 731 ± 621.12, were as in diabetes population it was 560.31 ± 319.81, and 629.37 ± 350.8 in both diabetic and hypertensive population. The mean and SD of D-dimer it was 3726.4 ± 2411.86 in diabetic group, 2861.28 ± 2041.36 in hypertensive group and 2755.6 ± 1980.67. CRP levels and D-dimer were positively correlated with mortality and duration of stay. Conclusion: Our study concluded inflammatory markers CRP and D-dimer levels were elevated in both comorbid patients and this was statistically significant. Correlation of ferritin to the outcome was not significant and understanding the molecular mechanism of infection in co-morbid patients and assessing the inflammatory markers can provide necessary assistance at earliest, for a better clinical outcome.

The surinfection of the renal graft by multi-resistant germs constitutes a serious complication, which can engage the functional and vital prognosis in renal transplant patients. It is responsible for a problem of therapeutic management and of the benefit/risk ratio between antibiotic therapy at therapeutic doses and the contraindications linked to renal insufficiency and therapeutic impasse. This work reports the case of a kidney transplant recipient who died following complications of a superinfection of the graft by a strain of Klebsiella pneumoniae producing OXA 48. The purpose of this observation is to underline the nosocomial risk in kidney transplant patients and the difficulty of the therapeutic management of these multi-resistant infections.

Analytical Studies on Retrofitted Anchorage System in Concrete using Strut and Tie Method

Analytical studies were conducted on force transfer mechanism of retrofitted anchorage system in structural concrete by Strut-and-Tie modeling (STM). Post Installation of Headed anchorage (PIHA) as supplementary system introduced for implicit strengthening of anchorage system. The boundaries of STM are considered under direct tension pull-out test. Five different configurations of conventional reinforcement anchorage in concrete with straight bar, 90-degree bend, 180-degree hook, single head and double head bars are retrofitted by using PIHA technique. The mechanics of force transfer in anchorage system was analyzed by STM and validated the results by experimental program. The study parameters considered are (i) location of nodal zone, (ii) strut angle, (iii) size of strut (concrete) contributed during failure. The study variables are (i) configuration of anchorage system (ii) characteristic node formation and (iii) presence of supplementary reinforcement. The result shows good agreement with experimental findings against failure mode, stress pattern, and location of critical zone in conventional and retrofitted anchorage system. Use of this study may further extended to assess theoretical evaluation of failure mode, formation of critical section and stressed regions of discrete RC elements such as corbel projection, bracket connections and beam-column joints.
